I'm on a family medical leave plan. It works for individuals as well as families. Just get a medical statement from your doctor, describing the nature of your illness and outlining how many days a month you may require to take off work. They won't describe in too much detail, but will usually say that you have a chronic condition that requires regular care. The doctors usually have their own forms as well as employers. My medical office used their own forms instead of the employer's forms. When you need to take off to deal with this problem, you can just ask your supervisor to take it off your Family Medical Leave, and it doesn't subtract from your regular sick leave or count against you, if you have a policy regarding too many absences. I have had to take a few days off or had to leave early. It depends on your state what plans they have in place. Hope this helps.
